Disability and Mental Health Referral Form
The Disability and Dyslexia Service offers confidential advice and support to students who have support requirements as a result of a recognised disability, including:
- mental health conditions
- specific learning differences (such as dyslexia, dyspraxia, and ADHD)
- autism and neurodiversity
- mobility impairments
- sensory impairments
- medical condition(s), unseen disabilities, or a temporary disability, such as injury

Confidentiality and disclosure
We encourage all students to disclose their support needs so that you can get the appropriate support to help you reach your full potential. If you do not disclose your support needs, then it can delay or even prevent you getting the support you need.
Any information you do share is kept confidential and only shared if you consent.The only time we may break confidentiality is if we feel you are at risk of harm to yourself or others.
You will not be discriminated against forsharing a disability. We will be able to provide additional support to meet your needs to allow you to reach your full potential.
