The Transition Hub research project is a feasibility study investigating whether adolescents new to care (or who experience a placement breakdown) can experience greater stability in their school and care placements as a result of a pilot Transition Hub research project.
Central to the programme is an understanding that transition is a process, rather than a one-off event. It recognises the interconnectedness of the young person, carers, school and community factors in supporting better outcomes young people in care and that three elements - ‘pupil ready, carer ready and school ready’ for transition are necessary for success and stability.
